Immediate Actions to Take Following an Incident

1. Confirm the well-being of all employees involved (Mammoet personnel and/or contractors).

2. Call to inform the applicable Project Manager and Supervisor

3. Call or text a SHE-Q Representative to inform them of the situation.

4. Call Cindy Mata (281) 299-5447, Vickie Rhodes (832) 986-0362, or Michelle Campbell (281) 309-2657 to set up post-incident drug screening.

5. For injury-related incidents:

a. Contact OptiCare Connect at (855) 644-6767 for medical case

b. If needed, call Cindy Mata, Vickie Rhodes, or Michelle Campbell to set up a medical evaluation at an OCC Med (the nearest Occupational Medical Center).

6. Vehicle-related incidents:

a. Exchange necessary insurance information & pictures of driver's license.

b. Call local authorities immediately.

c. Wait for the authorities to arrive

d. Obtain a police report (or report number).

e. Take detailed photos of all four sides of all vehicles involved.

7. Email notification:

Send Preliminary Incident Notification Form (PIN) email to PIN.AMER@mammoet.com.

For all incidents, email PIN Americas within four hours (SHE-Q will assist if necessary):

a. CC Project Managers and Supervisors

b. Use the following format and send to a SHE-Q representative (if template is unavailable):

i. Who: e.g. Mammoet Crane Operator

ii. What: e.g. “Crane Accident by failed rigging”

iii. When: Date and time of incident

iv. Where: Location of incident (e.g. ABC Plant, or XYZ Dock, City, State)

Note:

* The PIN should contain only factual information. Include relevant photographs as necessary, and only if content is appropriate.

* If access to the PIN Form is not available, use links listed in Documents section below.

8. Gather all necessary documents listed below and send them to Supervision and SHE-Q representative (or by clicking the links section below): 

a. Job Hazard Analysis (JHA)

b. Toolbox talks

c. Preliminary Incident Notification Form

d. Fill out an Insurance Form for ALL incidents involving vehicle/equipment damage where an insurance claim may be filed.

e. Equipment inspections

f. Permits

g. Witness statements

h. Pictures - Capture clear, detailed photos of the incident scene.

i. Drawings/sketches (as applicable)
